There were a lot of issues with Jedi Dogma at the time, but their mistake in letting themselves get drawn into the Clone Wars is what ultimately did them in.

I'd argue that the Jedi Order in spirit died 1000 years prior but hey, that's a discussion I'm not sure is appropriate here :P
