How can we do public health without thinking of people as units/populations? How can we avoid diagnoses / labels eg frailty when time-critical situations need succinct communication in handovers?

No-one with a LTC should present in a foreseeable crisis without a plan for action. Useful to guide patient & family, carers, first responders, ED etc, & plans made collaboratively respect the patient's values.

Very interesting to see actual evidence for it but I suspect not surprising to a lot of geriatricians.
Especially frustrating with bed pressures that you have medically fit patients with dementia/resolving deleroum moved to D/C wards waiting social care
Only for that move to trigger a deterioration
