I really enjoyed this last interactive session on the metaverse of ICU learning with Elena Sancho, Eleonora Balzani, and Pedro Povoa!

The quest for knowledge can go through many pathways. Probably the best is to integrate different modalities, adjusting them to specific needs.

Leo Celi gave a powerful talk about how to integrate AI in medical education: it requires redesigning of legacy systems.

We can’t simply rearrange the furniture after the Titanic has started sinking: end the Publish or Perish culture!

A system that can‘t be fixed with Al, can’t be fixed with Al.

We can get pervasive patient and environment information.

We can:
- Increase mobility
- Reduce Invasivity
- Improve data visualization
- Improve delirium recognition
- Bridge the gap between ICU and wards

Massimiliano Greco introduced wearable devices.

They come with challenges:
- Accuracy and validation
- Cost effectiveness
- Alarm fatigue
- Data visualization

The current model not well-suited if we frame healthcare (and ICU)
as a complex societal function to optimize health and well-being.

AI 3.0 is more than a product and it could probably disrupt the entire structure of healthcare.

On the final day of #ESICMInnovation Derek Angus talked about how AI could or should change medical education.

We should probably reconsider the core curriculum of medical learning.
Who is going to teach it? We need experts from other fields, such as statisticians and ethicists.

Nicole Hunfeld tackled pharmacogenomics!

We all tend to be oblivious of the genetic profile of our patients. In reality having the info about their cytochromes can really make a difference in their care.

A great example of the need to integrate different profiles in the ICU team.

Ari Ercole analyzed the neuromonitoring systems available.

He focused on the need to contextualize the data: the duration of insults, autoregulatory states…

You can now identify the optimal autoregulatory status for each patient.

The real future will be monitoring the doctor!

Kirsten Colpaert taught how AI-powered alarm management can reduce fatigue and
improve patient safety!

We need to educate patients, clinicians and families to better integrate the right AI model into our care.

Hence, most of the challenges are upstream of the data, and not enough hospitals are testing their predictive AI model for accuracy or data.

So… rather than thinking about new models, think how will humans change the way they practice due to the AI input.

We, as humans, are training AI to have biases.

The discussion is thriving around the concept of sustainability in four dimensions: environmental, economic, social, and operational.

If done correctly, it probably leads to a change in the structure of care!

Again, excellent speakers and faculty lead an exceptional and stimulating discussion at the end of each presentation!

Next, Ari Ercole discussed the feasibility of AI implementation in ICU.

The coolest, newest device is useless if it does not change the clinical practice.

Hence, we should focus on how to change our systems implementing AI for the benefit of our patients and not someone else.

While we could debate all night about how AI has its pros and cons, we should rather focus on the role of ESICM and other societies in developing adequate guidelines and regulations, benefiting humanity and limit damage.

During the second part, Elie Azoulay gives us the basis for ethical reasoning while interacting with AI.

We move around the concepts of fairness and truthfulness, balance of beneficience, maleficience and injustice.

I highly appreciate the style of the final discussions, that leaves a lot of space for the audience to interact with the speakers. We are hearing more about everyday aspects and reflecting about the real life impact of the decision that are still to be made on how to introduce AI in our field.

We will probably get to ULTRArapid diagnostic, precision medicine and novel therapeutics allowing for better targeting and medicine release. The final step will probably be AI driven CCSS.

AI will probably first reduce the physical workload, while boosting access to ICU and it will then augment ICU MD’s expert judgement. It will replace some jobs while accelerating renewal.

Regarding therapies, one good example comes from antibiotics…

The first half of the day introduced crucial concepts such as the value of lobbying and the need to not fear innovation but rather understand how and in which phases it will impact our jobs. Of course, there is no equity and no evolution without the right intention.
