I can't say for certain that this was the intent with the design of Cloud's Mom, I haven't read anything about it, but I think it works with the story.

So I think there are some story/design reasons for this. The game takes the time midway through to show Cloud's mind is influenced by Hojo's project, and is a deeply unreliable narrator. Tifa points out that Cloud wasn't at Nibelheim, though he does know a lot about the events that she doesn't understand how he would know. But he also lies openly about a lot of what happens there (some teasingly, like things he did in her house, most of them about the things the Soldier did). And a revisit to Nibelheim shows an entirely different family living in what he claims is his house. As the player you're supposed to question if anything he said about Nibelheim was true. And even the visit with his mom is told AFTER you've seen Cloud in a dress during the Don Corneo Mansion thing. So as the player you've seen Cloud in a dress, and his mom just happens to look almost exactly like Cloud in a different dress, which allows the player question his narrative of events even more.

Yeah, holding shift for focus mode will let you see the hitbox, which is a small centered red dot. I tend to just draw a mental box around the character and try to just focus on the bullets in that area, and only evade those unless if there is a pattern of fast scattering bullets
