If you have a passing interest in the UK’s experiment with a new model of care, the Leng review into Physician Assistants (ex-Associates) was published today in all 134 pages: assets.publishing.service.gov.uk/media/687760...

10/ And there are wider questions about how we ever got into this situation, introducing a new model of service delivery with so little thought. Some medical leaders have serious questions to answer, not least to those who have suffered as a result (both patients and PAs given false expectations)

🧵1/ Big changes are coming for the occupational group now termed physician associates (PAs) and anaesthesia associates (AAs) in England. Our new @bmj.com piece (@trishgreenhalgh.bsky.social & me) unpacks the implications of the Leng Review, commissioned by @wesstreeting. Here are our thoughts. 👇

Current selling point is ‘doctors won’t work here but PAs will’ speaks volumes to rural inequity and wondering how they will be supervised.

New Zealand to recognise physician associates - minister A newly-regulated healthcare role is hoped to unlock more pairs of hands for medical care, and attract more overseas workers to rural practices.

#missionimpossible: A medical perspective
- Attempted use of defibrillation without a rhythm check
- Tension pneumothorax less likely from a splenic gunshot
- Management of tension pneumothorax however most excellent
- Don’t leave your decompression chamber and immediately get in an airplane

My advance directives are pretty straightforward:

If I’m able to be saved, save me.

If I’m unable to be saved, don’t save me.

All you need to do is run my blood through the Can They Be Saved? machine and if the answer comes back “yes” then save me. If the answer comes back “no” then don’t bother.

🚨Medical device vulnerability: researchers found the Contec CMS800 Monitor contains a firmware backdoor.

If connected to the internet, the backdoor automatically connects to a specific IP & uploads patient info & sensor data.

It also allows remote code execution, which is potentially lethal!
