Table showing endotypes of insulin-glucose homeostasis in patients with hidradenitis suppurative (HS or acne inversa) and different expressions of haptoglobin concentration. In persons with HS and a high haptoglobin concentration, insulin sensitivity (SPINA-GR) and beta-cell function (SPINA-GBeta) are similar but combine to provide a reduced disposition index (SPINA-DI).

Plot of SPINA-GBeta versus SPINA-GR in patients with HS and controls: The majority of patients with HS and diabetes are faced with rather low insulin sensitivity and a lack of dynamic compensation via pancreatic beta-cell function, resulting in a reduced disposition index.

#Hidradenitis suppurativa (acne inversa) is linked to #diabetes mellitus. In the affected, #SPINA-GR is reduced and not compensated by #SPINA-GBeta, resulting in low #SPINA-DI. This phenomenon may be mediated by #inflammation, as shown by ties to #haptoglobin.

Graphical Abstract:

This novel fasting-based disposition index (SPINA-DI) is defined as the product of insulin receptor gain (SPINA-GR) multiplied by beta-cell function (SPINA-GBeta). SPINA-DI may be reduced due to insulin resistance with insufficient dynamical compensation of beta-cell mass (red horizontal arrow) or due to primary beta-cell failure (red vertical arrow).

SPINA-DI was validated in three cohorts. It had advantages in validity, reliability and diagnostic power compared to established criteria.

The disposition index of #insulin-glucose #homeostasis is an estimate for its loop gain. Its determination used to be complex, requiring clamp, IVGTT or OGTT experiments. We have developed a fasting-based method that is simpler and better πŸ§ͺ🩺

Simulation speed with implementations in different programming languages. Shown is the time needed to simulate a first-order linear feedback loop (A) and a zeroth-order MiMe-NoCoDI loop (B) dependent on the number of iterations (proportional to simulated time, abscissa) based on implementations of the simulation program and the CyberUnits Bricks library in S, Python, Swift, C++ and Object Pascal. The ordinate is logarithmically scaled and represents the mean of 10 replications of each number of iteration steps.

Which programming language is fit for high-performance computer simulations in biomedical #cybernetics? We found that #Free_Pascal generates faster code than S/R and #Python. Interestingly, Object #Pascal even outperforms #Swift and C++. πŸ§ͺ🩺

Simple and Reliable Early Prediction of Diabetes A simple blood test could perform better than a complex test thanks to mathematical modeling.

A novel method for early detection and #endotyping of #Diabetes mellitus relies on software written in #ObjectPascal.

Today, this innovative approach is covered by all major press agencies and news channels all over the globe.

4 Plots showing the hyperbolic relation between SPINA-GBeta (a measure for beta cell function) and SPINA-GR (reconstructed insulin receptor gain as a measure for insulin sensitivity).

(A) SPINA-DI remains constant as long as changes in insulin sensitivity (as measured by SPINA-GR) are balanced by adequate variations in beta cell function (SPINA-GBeta, circles 1, 2, and 3). (B) If dynamic compensation fails, either due to insufficient adaptation to insulin resistance (circle 4) or primary beta cell failure (circle 5), the disposition index breaks down and is markedly reduced. (C) Computer simulations with systematic variations of insulin receptor gain (GR) and secretory capacity of beta cells (GBeta) and resulting blood glucose concentration. (D) SPINA-GBeta plotted against SPINA-GR in the definition dataset. Results of persons with normal glucose homeostasis are near the hyperbola of normal SPINA-DI. The ability of dynamical beta cell compensation is partly lost in prediabetes.

Recently, computer simulations with #SimulaBeta played a key role in the development of the fasting-based disposition index SPINA-DI, a substantially improved method to assess the function of the insulin-glucose feedback loop and for early diagnosis of diabetes mellitus. 🩺 doi.org/10.1111/1753...
