A lot of the time, passing or failing also seems to depend on the whim of the individual clinical facilitator. They're often assigned to supervise students outside their area of clinical experience, and bring their own professional prejudices as a result.

I think a range of things. I think you could summarise it as workplace expectations not meeting reality.
Also the assessment process itself can be stressful and fraught. One mistake of the wrong kind and you've failed your 4 or 8 week placement, so it feels like you're walking on eggshells.

In every city. Good infrastructure gives way to scary, uncomfortable, dangerous. It's why people don't ride bikes everywhere. They don't line having the sh!t scared of of them. Also they don't want their 5 & 12 year olds to get killed, nor their grandparents.
