Well, they just kept saying "them" - "hit them hard," "take out their capabilities," etc. It wasn't clear to me whether they had decided it was a particular country or had decided (against all logic, evidence, and common sense) that China, Russia, and North Korea constituted a unified threat.

Unless you think it's remotely realistic that Russia, China, & N. Korea would all join forces to launch an incredibly risky nuclear attack--which, by the way, they certainly couldn't count on the US being unable to identify the source of, as happens in the movie.

Yes, this is it exactly - the whole "dilemma" is built on a Cold War premise where nuclear war is essentially a binary contest. There *was* a certain grim logic to M.A.D., but that game theory's out the window in the movie's scenario.
