We stopped sending SWAT to barricaded suicidal subjects *unless there were third parties in danger* because data show it increases the likelihood of a lethal outcome.

With the disclaimer that I don't know the specifics of this case, I've been called to similar scenes in the past when working with SWAT.

Someone threatening suicide by lethal means (usually a firearm) can induce law enforcement to shoot them by threatening others. "Suicide by cop."
